Research Report Evaluation of the Multi-Payer Advanced Primary Care Practice (MAPCP) Demonstration: Second Annual Report

Through CMS’s MAPCP Demonstration, which began in 2011, certified medical home practices in eight states received monthly care management fees from Medicare, Medicaid, and private payers that were designed to average $10 or less per patient per month, plus additional resources (e.g., quality measure reports, learning collaboratives). By the second year, this demonstration included 5,200 health care providers in 800 practices and 600,000 Medicare beneficiaries, and had produced net savings for Medicare of $324 million. Interviewees valued the demonstration, the medical home model, and the care managers that practices had hired to manage high-need patients, but expressed disappointment with demonstration payment amounts.
