Research Report Evaluation of the Multi-Payer Advanced Primary Care Practice (MAPCP) Demonstration: Third Annual Report

The multi-payer MAPCP Demonstration began in 2011 and provided certified medical home practices in eight states with monthly care management fees designed to average $10 or less per patient per month, plus additional resources (e.g., quality measure reports, learning collaboratives). By the third year, this demonstration included 6,400 providers in 850 practices and 700,000 Medicare beneficiaries. Interviewees reported that practices were comfortable with the basics of the medical home model (e.g., using full-time care managers to target high-risk patients and a team-based approach to care) and were making minor refinements (e.g., modifying care manager roles, customizing EHRs). A separate final report will present impact analyses of claims data and findings from focus groups with patients.
