This third annual report on the long-term evaluation of the Medicaid health home option focuses on providers’ experiences with the new model of care, created in the Affordable Care Act (ACA). The Urban Institute is conducting the five-year evaluation for the Department of Health and Human Services (HHS) Assistant Secretary of Planning and Evaluation (ASPE). The health home model focuses on high cost, high need populations; the integration of physical and behavioral health care; and care coordination and management that extends beyond medical services to include nonclinical supports, such as transportation and housing. This report examines implementation challenges, provision of required services, and use of health information technology (Health IT) in the first 11 states with approved programs: Alabama, Idaho, Iowa, Maine, Missouri, New York, North Carolina, Ohio, Oregon, Rhode Island, and Wisconsin.
